Quod nemo – preter gremialem apothecarium Alexandrum Peak – contra me querelam coram Amplissimo Magistratui a tempore habitationis hic loci mea – deposuerit – quodne me mori gerum excubuerim testis sub civitatis hujus Libere Regieque sigillo mihi expediri humillime supplico – plena cum veneratione perseverando.
